SHINING STAR CHILD CARE 2010 Enrollment and Termination Policy
A non-refundable registration fee of $75.00 and the first payment is due and payable upon signing the enrollment agreement. All forms must be filled out completely and returned before childcare is to be provided. Such forms include but are not limited to:
1 Identification and Emergency Information 2 Immunization Record 3 Consent for Medical Treatment 4 Liability Insurance Affidavit 5 Parent’s Rights and Personal Rights 6 Authorization to Transport 7 Authorization to Administer Medication
All forms must be kept up to date and are subject to an annual renewal every September and January. Please notify the provider immediately if there are any changes to work, cell phone, pager numbers, etc., so that the child’s records are kept current.
Appropriate placement of a child is very important. Every program is not suited to every child therefore, all children are allowed a two-week trial period to ensure proper placement. During this period, either party may cancel this agreement without notice. After the trial period, the parent must give a two-week written notice. Parents may choose to pay for two weeks worth of childcare in lieu of not returning to day care upon notice to provider.
A parent/provider agreement may be terminated at any time, by the parent/provider only if it is felt that the continued care of a child would be detrimental to the provider’s childcare program. Only in this instance will pre-paid tuition fees be refunded for those days when care will not be provided.
Provider reserves the right to terminate agreement at any time upon provider’s own discretion.
Payment Policy
Payment obligation is based upon the hours agreed to attend day care, not actual hours of attendance. All fees will be paid with cash or check and noted with a receipt. In the event that a check is accepted in lieu of other form of payment and returned unpaid, a $30.00 returned check fee will be due and payable. Provider will require payment in cash after the second time that a check is returned unpaid. There will be no credit refund for holidays, absences, and incomplete attendance of scheduled hours, late fees, parent’s vacations and/or any instances when child(ren) may be sent home due to health reasons. Payments not received by the due date must include a $5.00 per day late fee. If payment is not received when due, the child(ren) may be dismissed and may not be allowed to return to day care until all payments due are made and only if there is an opening available. Pick-up times must be strictly observed. Parents must pay additional fees when children are left at day care at times not included in their care schedule. The charge for additional care requested, not per the schedule, is at a rate of $3.00 per hour, per child. Payment for such is due and payable upon pick-up of the child(ren). Notice must be given as soon as possible for additional care requests and must be approved by provider. A fee of $8.00 will be due and payable upon pick-up of children one minute after the scheduled pick-up time up until ten minutes after, as well as every fifteen minutes (or any part of the fifteen minutes thereafter). Late pick-up is grounds for termination of child care services without advance notification. Normal hours of operation usually exceed nine hours per day. Parent is asked to please be considerate of provider’s personal schedule. The day care is the provider’s employment, which is located at provider’s residence. Provider looks forward to family and personal time the same way that parents look forward to their family and personal time when it is time to clock out of their job. For this reason provider asks parents to be on time.
Parent must give two weeks notice for any schedule change. Schedule changes require a new daycare contract.
Health Policy
It is important that every parent cooperate with the day care health policy. Provider must be informed if child(ren) or family members has/have any communicable disease(s). Regulations are designed to protect the well being of all children in day care and to guard as much as possible against avoidable absences for health reasons.
Provider must be informed if the child has been ill during the previous weekend, evening before, or the morning of returning to day care. When there are symptoms of illness or indications that a child is not well, such as fever or the first two days of a bad cold or cough, arrangements must be made for the child’s care at home or alternate location for the sake and well-being of other children, parents and provider.
An authorization to administer medication is available for medication that the parent wants provider to administer to the child. Medication will be given only if it is in the original container with prescription label attached.
Should parent observe any of the following symptoms arrangements for alternate care must be made:
1 An oral temperature above 100° 2 Nausea or vomiting 3 Stomach cramps with diarrhea 4 Earache 5 Sore throat 6 Excessive runny nose 7 Excessive coughing 8 Symptoms of communicable diseases or infections
Day care can be a safe place for well children only when children with signs of illness are identified promptly and separated immediately from the other children. If any signs of illness are present and parent has questions about whether or not the child(ren) may attend day care parent must call provider for approval of attendance.
Parents occasionally miss signs of their child(ren) not feeling well. If provider notices symptoms of illness and notifies parent that their child is ill, parent must pick up their child(ren) promptly or arrange for child pick-up by contacts listed in the Identification and Emergency Information list. Ill children may be separated from children that are well, until parent arrives for child(ren).
Parents will be notified immediately of any serious accident. In the case of an emergency and if parent(s) cannot be reached, the physician on the emergency contact form will be contacted if time permits. A signed consent for emergency treatment should be made available to the child’s physician. Parent is advised to supply a photocopy of the insurance card for the child’s daycare file.
Meals and Snacks
Provider will serve meals per the following schedules: 1 Breakfast 6:45 2 Lunch 11:30 3 Snacks 3:00
If child decides not to eat from the planned menu that has been prepared at the time it is served the child will be offered something to eat at the next scheduled planned meal or snack.
There are times when the provider encounters finicky eaters. Although provider would love to please children with their choice of food, provider will only serve what is prepared for the meal. If parent finds that their child(ren) is not eating the food being prepared, parent may supply child with food from home only if it is considered healthy and nutritious and does not require the provider to prepare the food. Cereals that are high in sugar, or junk food are not considered to be healthy choices for meals. The following meal examples are supplied to help parents know what to bring for their child’s meal replacements.
Breakfast: Milk Fruit, vegetable, or 100% juice Bread group (Cereal, waffles, pancakes, muffins, etc.) Optional other (eggs, ham, bacon, etc.)
Snack (2 out of 4): Milk Meat or Alternate (protein group such as peanut butter, cheese, yogurt, nuts, etc.) Fruit, vegetable, or 100% juice Bread group
Lunch: Milk Meat or Alternate Vegetable or Fruit Vegetable or Fruit Bread group (tortilla, rice, pita, muffin, etc.)
Children have a tendency to tell parents upon pick-up that they are hungry. Provider will always offer the child(ren) something to eat at the scheduled snack and meal times. If child(ren) say they did not eat it is because they chose not to eat at the scheduled times. Parent(s) are asked to remind the child(ren) about that general rule.
Birthdays and holidays are special days for children. Parents are encouraged to bring special treats for all the children to celebrate those occasions.
Field Trips
Provider will have children participate in several field trips throughout the year. Certain field trips will have different age requirements allowing non-age appropriate children to participate in different activities. Additional fees for the field trips such as admission fees are not included in the tuition rate. Provider will give advance notice as to the amount that is due for such trips.
All persons participating in any field trips planned shall be deemed to have waived all claims against all staff at Shining Star Child Care for injury, accident, illness, or death occurring during or by reason of the field trip or excursion.
At times provider will require parents to supply child(ren) with sack lunches for field trips.
Clothing and Supplies
Parent is to send child(ren) to day care with proper attire at all times. Child(ren) must not be brought to day care in their pajamas. Parent is encouraged to supply older child(ren) with an extra change of clothing if parent finds it necessary. Infants and toddlers are required to bring extra clothing.
Undergarments such as disposable diapers or pull-ups for potty training and diaper wipes must be supplied as well for children who need them.
A washable pad or pillow and small blanket is requested for small children in day care from 12pm - 3 pm for naptime.
All of the child(ren)’s belongings should be labeled with their name. Provider is not responsible for lost or broken items.
Survival Packets
In the event of a disaster during daycare hours provider wants to be prepared. A 72-hour non-perishable food packet is required. Parent should supply the packet as soon as possible. All items must be stored in a one-gallon zip lock plastic bag. Only one bag per child is required. The packet should have a piece of paper with all vital information taped facing out on the inside of the bag. Information should include parent’s name, child’s name, health conditions, medications needed, etc. The packet should have individual serving size food items. The following guidelines will assist parents in the preparation of the child(ren)’s survival packet:
Infants: 1 Powdered milk 2 Baby Cereal 3 Extra diapers 4 Wipes and ointment
Toddlers & Children: 1 Dried fruit or nuts, 2 Fruit rolls 3 Beef jerky 4 Granola mix 5 Granola bars, 6 Food bars 7 Small pop-tops cans of fruit, pudding, tuna, etc. 8 One or two 6-8 oz. Of canned juice 9 Plastic spoons and or forks 10 A picture of parent(s), and/or a small, personal, treasured keepsakes, which child(ren) can identify with during a crisis
Water will be provided by and stored at the day care.
Glass, easily damaged items or both items will not be accepted.
Miscellaneous
The provider will discipline the child in any manner deemed necessary, that which is not abusive and does not include corporal punishment. If a child is behaving unacceptable, the daycare exercises “time-out” and sends the child to a quiet spot where there are no distractions for this purpose. If need be, a provider/parent conference will be scheduled so that everyone can work together to improve the child’s behavior. If it is felt that the child’s behavior is not improving and the child is not able to follow rules, the provider reserves the right to dismiss the child from the day care with no advance notice.
Parents are responsible for their children’s behavior when parents are in the facility. Parents are to make sure children are not running, jumping, climbing, or doing anything else that may cause injury to themselves or disruption to others.
Subsidized Child Care Programs
Parents who have their childcare paid through state funded programs are responsible for any fees that the state does not pay for. This will include any fees that are not paid due to holidays, absences, and incomplete attendance of scheduled hours, late fees, parent’s vacations and/or any instances when child(ren) may be sent home due to health reasons. The current rate that the parent will be responsible for is $_________ per day.
Child(ren) must be signed in and out on a daily basis.
Photograph Permission
The Provider occasionally will take pictures of the children. The pictures are used for memoirs, scrapbooks, as well as for the Shining Star Child Care web site. Occasionally provider will use pictures for advertising purposes on fliers.
